Annual Food Service Establishments
2023 Annual Food Service Permits expire December 31, 2023
2024 Annual Food Service Applications are due by December 12, 2023 at 7:00 pm to the Board of Health office.
- Annual food service applications submitted later than 7:00 pm on Thursday, December 14, 2023 will be subject to the $25 late fee on top of any permitting fees.
- Permits can be submitted during Town Hall Hours, by mail, or in the drop box outside Town Hall anytime.

Temporary Food Service Permits
Event permits: Temporary Food Service Application (includes vendors and food trucks)
All applicants MUST submit application and fee 30 days prior to the event.
Event Coordinator's checklist (for multi-vendor events)
Must be submitted by the event planner or coordinator for multi-vendor events (festivals, fairs, etc.) at least 30 days prior to the event. Please include a list of all vendors who will be attending the event. Multi-day events must submit a checklist for each specific day of the event.
Caterer's Notification: Please include copies of your home municipality Food Service permit; servsafes certificates; menu for the event; copy of liability insurance; copy of worker's compensation insurance. Must be submitted at least 30 days prior to event.

For those vendors who plan on doing multiple events (3 or more) during any given four month period, please submit a Season Food Service permit.
