Annual Food Service Establishments

2018 Annual Food Service Permits expire December 31, 2018
2019 Annual Food Service Applications are due by December 20, 2018 to the Board of Health office.

Annual application for all restaurants, food retails, and temporary food service based in Wenham.

For new food service establishments: Food Service Establishment Plan Review Application

Temporary Food Service Permits

Event permits: Temporary Food Service Application (includes vendors and food trucks)
All applicants MUST submit application and fee 30 days prior to the event.

Event Coordinator's checklist (for multi-vendor events)
Must be submitted by the event planner or coordinator for multi-vendor events (festivals, fairs, etc.) at least 30 days prior to the event. Please include a list of all vendors who will be attending the event. Multi-day events must submit a checklist for each specific day of the event.

Caterer's Notification: Please include copies of your home municipality Food Service permit; servsafes certificates; menu for the event; copy of liability insurance; copy of worker's compensation insurance. Must be submitted at least 30 days prior to event.

Seasonal Food Service Permits 

Seasonal Food Service Permits:
For those vendors who plan on doing multiple events (3 or more) during any given four month period, please submit a Season Food Service permit.
